You will need to return this form to an MNCPA staff member following the closing session.

Please note: Approval from the Minnesota Department of Commerce may still be pending at the time of the seminar/conference. If this is the case, attendees who signed up for Minnesota Insurance credit will be notified by the MNCPA following the seminar/conference regarding approval or denial.
How Minnesota Insurance credits are reported
- For approved events, the MNCPA will report the continuing education credits to the Minnesota Department of Commerce via the Sircon database one to two weeks after the event, or when approval is received from the Minnesota Department of Commerce (whichever is later). Credits will be reported only if you follow the requirements shown above.
- After the continuing education credits have been processed, the MNCPA will mail each attendee a certificate of attendance showing the course approval number, number of credits earned, etc.
Please Note: Effective August 1, 2010, the Minnesota Department of Commerce will no longer be accepting individual applications for CE course approval. For more information, please contact the Minnesota Department of Commerce at 651-286-6319 or refer to their website.
