The following message is
from the Minnesota Department of Employment and Economic
Development (DEED).
The error
Payments of unemployment benefits (UI), disaster unemployment
assistance (DUA), and Trade Act benefits (TRA) that were mailed
to unemployment benefit recipients (applicants) during the week
of January 14, 2008 contained a printing error.
1099-G forms sent to applicants who had optional withholding
for state (SIT) and federal income taxes (FIT) correctly show
the FIT in box 4. However, the SIT was incorrectly printed in
box 2. That amount should have appeared in the box to the right
of the Department's State Income Tax Withholding ID Number in
the lower left quadrant of the document, and been identified as
"State Tax Withheld", as shown in the image below:

DEED action
DEED is developing a plan to
reprint and mail corrected 1099-G forms to all applicants who
were affected by this error. However, tax practitioners may see
some of these individuals before the corrected documents are
mailed, or with the original incorrect 1099-G, the correction
letter and/or the corrected 1099-G. The amounts sent to the IRS
and Minnesota Department of Revenue electronically were correct.
Only the paper forms that were mailed to applicants were
affected by the error.
